Post Mission Debrief: That Akarist is a Tricky one - he managed to broker a deal with the entity I made the pact with! Still, I am free of my debt, so all is well.

Cave/Temple with CoT. If you die inside and click 'Go to Hospital' it will port you to the far side of the map inside a small room with a 'magical barrier' over the door that you can destroy to exit the room. Be aware this Barrier has a LOT of HP and damage resistance and you are unable to teleport out of the room as it puts a 'Dampening field' debuff on you. The map is HUGE with lots of aggro from on top of buildings and under floors. The altar is a clickable small thing that for me was hidden behind a pillar.
